Making an Appointment

To make an appointment with one of our providers, please contact us by phone (303) 840-5051 or fax (303) 840-5058 with the following information:

  1. Referring physician name and a written referral if required by your insurance.

  2. Insurance information.

  3. Reason for consultation.

Initial Consultation

For your initial consultation, please arrive 15 minutes before your appointment, and bring the following if applicable:

  1. All recent medical records and laboratory studies.

  2. All radiographic images and their reports.

  3. A list of all medications or the actual medications.

  4. Names of previous physicians seen for the current neurological issue.

  5. To simplify your first appointment please complete the new patient paperwork prior to your appointment. You can bring a copy of the completed paperwork with you or email us at admin@neurologyoftherockies.com.

It is the patient’s responsibility to bring in all records and proper information to maximize clinical care. If proper information is not brought to the visit, this may delay the clinic appointment, diagnosis, treatment, or cause duplicate testing.

Prescription Refills

  • Please call your pharmacy first for medication refills.

  • Prescription refills are authorized only during normal business hours Monday to Thursday, and Fridays from 8:30 a.m. to 12:00 p.m.

  • Prescriptions will not be filled after hours or on the weekends or holidays.

  • We require 24-48 hours for prescription refill authorizations.

  • Please notify your pharmacy at least 5 business days before you are out of your medication.
